Hormonal belly refers to abdominal weight gain caused by an imbalance in hormones. It typically affects women, especially after menopause. It is not a distinct medical condition and most of the time, the weight gained around your waistline is not always caused by hormones. As everyone’s body is different, it is often a combination of factors which contribute to how fat is distributed.

However, if you have suddenly gained weight and with expanding waistline that does not seem to link to your nutrition, sleep or fitness habits, you may be having hormonal issues.

How do I know if I have a hormonal belly?

Many things can lead to abdominal weight gain. It may not be possible for a person to find out if they have a hormonal belly on their own.

When hormonal imbalances lead to abdominal weight gain, fat builds up around your internal organs, known as visceral fat. This causes your belly to expand, giving it an apple-shaped appearance. Instead of simply looking overweight, you may appear noticeably bloated.

Hormonal belly may appear regardless of diet and exercise, and you may be confused when you keep gaining extra inches around your belly even if you practice a balanced diet and exercise regularly.

However, if weight gain or bloating happens around menstruation, it only occurs around the abdomen and is not due to changes in diet or physical activity; the cause could be hormonal.

A doctor can determine if a person has a hormone imbalance by performing tests.

What causes hormonal belly in women?

Some potential causes of hormonal belly in women include:

polycystic ovary syndrome (PCOS)

Menopause

Period-related Fluid-retention

Polycystic Ovary Syndrome (PCOS)

According to the World Health Organisation, Polycystic ovary syndrome (PCOS) affects an estimated 6–13% of reproductive-aged women and up to 70% of affected women remain undiagnosed worldwide.

Women with PCOS tend to have higher levels of male hormones (androgens) and elevated insulin levels, which influence how the body converts food into energy. This can lead to weight gain, particularly in the abdominal area.

PCOS makes it harder for the body to process insulin, the hormone responsible for converting sugars and starches into energy. This condition, known as insulin resistance, leads to a buildup of insulin and glucose in the bloodstream.

Elevated insulin levels stimulate the production of androgens, or male hormones, which contribute to symptoms such as excess body hair, acne, irregular menstrual cycles, and weight gain. Since androgens influence fat distribution, women with PCOS often accumulate weight in the abdominal area, resembling an “apple shape” rather than the typical “pear shape.”

Abdominal fat is particularly concerning because it is linked to a higher risk of heart disease and other health complications.

Menopause

Menopause is when you stop having periods and your ovaries stop making estrogen. As women age and estrogen, the female reproductive hormone, is depleted, there can be changes to the metabolism of fat that can lead to belly fat gain.

Menopause weight gain can have serious effects on your health. Extra weight, especially around your midsection, increases your risk of many health issues, including:

Breathing problems.

Heart and blood vessel disease.

Type 2 diabetes.

Period-related fluid retention

Some people retain more fluid in the body during their period. This can cause bloating, especially around the stomach, which may look like weight gain.

In the days before your period, estrogen and progesterone rapidly decrease. This tells your body that it’s time to begin menstruation. Estrogen and progesterone also control the way your body regulates fluid. When these hormones fluctuate, the tissues in your body accumulate more water. The result is water retention, or edema.

Water retention may cause swelling or puffiness in your breasts, stomach, or extremities. This increases body weight, but not fat.

When to see you doctor?

If your hormonal belly is accompanied with the symptoms below, please seek immediate help from your medical provider.

Sleep problems

High stress

Appetite changes

Hair growth or hair loss

Bowel symptoms

Abdominal pain

Are you concerned about hormonal belly?

You may book an appointment with any of the hospitals or clinics in Malaysia to seek for pregnancy care.

Need help to find your healthcare provider?
